Continuity of care is essential for the patient transitioning from the hospital to a lower level of care. This continuity helps prevent potential costly compliance and care plan failures, reduces patient readmissions and improves clinical outcomes. Our surgeons monitor and treat patients at all post-acute settings. We preventively address wound and surgical site concerns and treat issues that otherwise would require an ER or hospital visit.
